Who do I go to if a Business/Staffing Firm in NJ won't cooperate?

I am going on unemployment in NJ and I need the dates I started work with the staffing firm for my interview with the state. I called the woman whom I worked for and she refuses to give me the dates and is ignoring me. She said she mailed it in to the state but just encase the letter doesn't reach them I need the dates over the phone and Ive been waiting 2 months for this interview with the state! Seriously what hatred!
